Where Evidence Is Strongest
The NIH Office of Dietary Supplements reports that creatine monohydrate can improve performance in repeated short bursts of high-intensity activity and can support some resistance-training outcomes. Research protocols often use either a short loading phase followed by a smaller maintenance amount or a steady daily amount without loading. Those study protocols are not a personalized dose recommendation. Body size, diet, health conditions, medicines, product quality, and training goals all matter.
Limits and Safety
Water-related weight gain is common. Gastrointestinal symptoms are also reported. Evidence summarized by NIH supports use for several years in many healthy adults, but that does not establish safety for every person, during pregnancy or breastfeeding, or in the presence of kidney disease or interacting medicines.

A 2025 systematic review and meta-analysis found a modest increase in serum creatinine but no statistically significant adverse change in glomerular filtration rate. The authors also cautioned that the evidence is dominated by relatively small studies and that long-term data beyond one year are sparse. That supports reassurance for healthy adults using standard protocols, not a promise of zero risk for every person or every dose.
Tell the clinician interpreting your blood work that you take creatine. They can decide whether the result needs another marker or assessment rather than assuming a creatinine-based estimate tells the whole story. Existing kidney disease, an unexplained abnormal kidney result or medicines that affect kidney function are reasons to ask a clinician before supplementing, not to self-test with a loading phase.
Do you need to load, cycle or time creatine around a workout?
Loading is optional. The NIH Office of Dietary Supplements describes two common research protocols: about 20 g per day split into four doses for five to seven days followed by 3–5 g per day, or roughly 3–6 g per day for three to four weeks without loading. Both are ways researchers raise muscle creatine; loading reaches saturation sooner but is more likely to cause a rapid scale increase or gastrointestinal discomfort.
There is no established need to cycle on and off. Creatine works by increasing tissue stores, so regular intake matters more than attaching it to a narrow post-workout window. Taking a modest daily amount at a time you reliably remember is a simpler default than repeatedly loading.
These are study protocols, not personalised dosing instructions. Body size, age, medical history, pregnancy, other supplements and medications can change what is appropriate. More is not automatically better, and a heaped kitchen spoon is not a reliable measuring device.
Is creatine monohydrate better than more expensive forms?
Monohydrate is the comparison standard because it has the deepest evidence base. NIH notes that creatine ethyl ester, buffered forms and other usually more expensive products have not been proven superior for raising muscle creatine, performance, stability or safety. A novel delivery system can change marketing without demonstrating a better outcome.
Choose a product that lists the amount of creatine monohydrate per serving and has credible independent quality testing. Supplements do not receive the same premarket approval as medicines, so purity and label accuracy are separate questions from whether creatine itself works. A multi-ingredient "performance blend" also makes it harder to know which ingredient caused a benefit or side effect.
What benefits are well supported and which remain uncertain?
The strongest case is for repeated short, high-intensity work and the training adaptations that follow it. NIH summarises numerous trials showing potential improvements in strength, power and work from maximal muscular contractions, while noting little value for endurance performance. Claims about cognition, sleep deprivation, mood, healthy ageing and treatment of disease are active research areas with different levels of evidence. They should not be bundled into a guarantee that everyone will feel mentally sharper or recover faster. A benefit observed in a specific population or laboratory task may not generalise to every healthy user.
Water drawn into muscle can raise scale weight early. That is not the same as gaining fat, but it matters if you compete in a weight-class sport or use short-term scale change as your only progress measure. Track the trend and the outcome you care about—training performance, strength or tolerance—rather than assuming every kilogram is new muscle.

Ask a qualified clinician or pharmacist first if you have kidney disease, abnormal kidney tests, are pregnant or breastfeeding, are under 18, or take medication that can affect renal function or fluid balance.
