Total, Added, and Free Sugars

These terms are not identical. FDA labels show Total Sugars and the amount that Includes Added Sugars. WHO's free sugars category also includes sugars in honey, syrups, fruit juice, and juice concentrate, while it does not include sugar within intact fruit or milk. See Sources 1 and 2 below.

Reference Guidance

The FDA Daily Value for added sugars is 50 grams on a 2,000-calorie reference diet. WHO recommends keeping free sugars below 10% of total energy, with a conditional suggestion that below 5% may provide additional benefit. These are population references; an individual's calorie needs may differ.

Practical Ways to Reduce Added Sugar

  • Read the serving size and “Includes Added Sugars” line.
  • Compare similar drinks, yogurt, cereals, and sauces using the same serving size.
  • Choose water or unsweetened drinks more often.
  • Keep naturally nutrient-rich foods such as intact fruit in context rather than treating all sugar grams as equivalent.

Avoid Oversimplified Claims

One sugary food does not directly become visceral fat, and an insulin response is a normal part of metabolism. Long-term dietary pattern, total energy, dental exposure, and health context matter more than a single item.

Free sugars, added sugars, total sugars — which number matters?

Three terms get used interchangeably and they are not the same, which is why label reading so often produces the wrong conclusion.

Total sugars is everything: the lactose in milk, the fructose in an apple, and the sucrose stirred into a biscuit. It is the only figure most labels outside the US declare.

Added sugars is what went in during manufacture or cooking. The US Nutrition Facts panel separates this out as "Includes X g Added Sugars".

Free sugars is the category WHO's guideline actually uses, and it is slightly wider than added sugars: it covers added sugar plus the sugar naturally present in honey, syrups and fruit juice. It excludes sugar in whole fruit, vegetables and milk.

WHO makes a strong recommendation to keep free sugars under 10% of daily energy and a conditional one that below 5% — around 25 g a day — is better still.

The practical consequence is that total sugar on a label routinely misleads in both directions. Plain yoghurt showing 8 g of sugar per 100 g contains essentially no free sugar; it is all lactose. A "no added sugar" fruit juice can carry more free sugar per 100 ml than a cola, because juicing releases sugar from the fruit matrix and WHO counts it regardless of the fact that nobody added it.

Where a label declares only total sugar, the ingredients list is the only way to tell them apart.

Is sugar addictive?

The comparison to addictive drugs is popular, frequently overstated, and the honest position sits between the two camps.

What is supported: sugar activates dopaminergic reward pathways, and highly palatable foods combining sugar with fat and salt drive consumption well past energy need. Some people experience genuine loss of control around these foods, and binge eating disorder is a recognised clinical condition.

What is not well supported: that sugar produces dependence in the way nicotine or alcohol does. The animal studies most often cited to show "sugar addiction" produced addiction-like behaviour under intermittent access — where the sugar was withheld and then returned. Under continuous access, the behaviour largely disappeared. That detail matters, because it suggests the restriction pattern itself contributes to the binge behaviour.

The practical implication runs against the usual advice. If restriction-then-access is part of what drives loss of control, then total elimination is a poor strategy for most people — it constructs exactly the intermittent pattern that produces the problem. Reducing gradually while keeping some sweetness available tends to work better than a ban, which also matches what happens in practice: bans produce a fortnight of compliance followed by a rebound.

There is a real effect people often mistake for withdrawal in the first days of cutting back — headache, irritability, low energy — which is more plausibly the combination of changed eating patterns, lower total calories and habit disruption than a pharmacological withdrawal syndrome.

Are artificial sweeteners a good substitute?

They are a reasonable transitional tool and a poor permanent identity, and the evidence is genuinely mixed rather than settled either way.

In their favour: substituting sweeteners for sugar in drinks does reduce calorie intake and modestly aids weight loss in controlled trials. For someone drinking a litre of cola a day, switching is a substantial improvement on doing nothing, and the dental benefit is unambiguous.

Against: WHO issued a conditional recommendation in 2023 against using non-sugar sweeteners for weight control, on the basis that long-term observational data do not show sustained benefit and suggest possible associations with adverse outcomes. Observational data on this question is badly confounded — people already gaining weight switch to diet drinks — but the recommendation exists and is worth knowing about.

The palate argument, which is weaker than commonly claimed: the idea that sweeteners maintain a sweet tooth and drive cravings is intuitive but not well demonstrated in controlled studies.

A defensible position: use them to get off sugar-sweetened drinks, which are the single largest source of free sugar for most people, and treat that as a step rather than a destination. Water, sparkling water, unsweetened tea and coffee are the endpoint.

How much sugar should you eat per day?

WHO recommends keeping free sugars below 10% of daily calories, with a further benefit below 5%. On a 2,000 kcal diet that is 50 g at the upper limit and about 25 g as the better target. Free sugars include anything added during manufacture or cooking, plus honey, syrups and fruit juice — but not the sugar in whole fruit, vegetables or milk.

Is fruit sugar bad for you?

No. WHO explicitly excludes sugar in whole fruit and vegetables from its free-sugars limit, because it arrives with fibre, water and micronutrients that slow absorption and blunt the glycaemic response. Fruit juice is treated differently and does count, since juicing removes the fibre and releases the sugar.
