Over 700 peer-reviewed studies. Consistently safe. Consistently effective. If there's one supplement with an airtight evidence base, it's creatine monohydrate. Yet myths and misunderstandings persist — it's "only for bodybuilders," it damages kidneys, it's basically a steroid. None of these are true.
What Creatine Is
Creatine is a naturally occurring compound synthesized in your liver and kidneys from the amino acids arginine, glycine, and methionine. Your body makes about 1–2g per day. You also get small amounts from meat and fish (roughly 1–2g per 500g of red meat). But to meaningfully elevate muscle creatine stores, you need to supplement.
Creatine is stored in your muscles as phosphocreatine. During explosive, high-intensity efforts — a heavy squat, a sprint, a max-effort jump — your muscles need ATP (adenosine triphosphate) faster than the aerobic system can supply it. Phosphocreatine rapidly donates a phosphate group to ADP, regenerating ATP in milliseconds. This is the phosphocreatine energy system, and creatine is its rate-limiting substrate.
What the Research Actually Shows
Strength and power output — Creatine supplementation consistently increases maximum strength by 5–15% and power output by 10–20% across dozens of studies. The effect is most pronounced in short-duration, high-intensity efforts.
Lean mass — Users typically gain 1–3kg of body weight in the first week (water drawn into muscle cells as creatine is stored). Over months of training, creatine users gain significantly more lean muscle mass than placebo groups.
Recovery — Creatine reduces exercise-induced muscle damage markers and accelerates glycogen resynthesis post-exercise. You can train more frequently because you recover faster.
Cognitive function — This is underappreciated: the brain also uses phosphocreatine. Multiple studies show creatine improves working memory, cognitive processing speed, and mental fatigue — particularly in sleep-deprived individuals or vegetarians whose baseline creatine stores are lower.
Aging — Sarcopenia (muscle loss with aging) is a major driver of frailty. Creatine combined with resistance training in adults over 55 consistently outperforms training alone for preserving muscle mass and strength.
How to Take It
Dose: 3–5g per day of creatine monohydrate. That's it. Timing doesn't matter much — consistency does.
Loading phase (optional): 20g/day split into 4 doses for 5–7 days, then 3–5g/day maintenance. Loading saturates muscle stores in about a week. Without loading, you reach the same saturation in 3–4 weeks. Loading causes more initial water weight gain and some GI discomfort in sensitive individuals — it's not necessary.
Form: Creatine monohydrate is the gold standard. It has the most evidence, the lowest cost, and performs identically to fancier forms (creatine ethyl ester, Kre-Alkalyn, creatine HCL) that cost 5–10x more. Don't be upsold.
With what: Creatine is absorbed fine with water. Pairing it with carbohydrates (which spike insulin) may marginally improve uptake, but the effect is small. Take it whenever is convenient for you to be consistent.
Cycling: Not necessary. There's no evidence that cycling on and off improves outcomes or prevents adaptation.
Safety
Creatine has been studied intensively for over 30 years, including long-term use, and no credible study has found kidney or liver damage in healthy individuals. The kidney concern arose from creatine raising serum creatinine (a kidney marker) — but creatinine is a breakdown product of creatine metabolism and is naturally higher in people with higher muscle mass. It doesn't indicate kidney strain.
Who should consult a doctor first: individuals with pre-existing kidney disease, as the clearance of creatine metabolites may be affected.
Non-Responders
About 25–30% of people don't respond significantly to creatine supplementation. This is typically because their baseline muscle creatine stores are already high — often the case in people who eat large amounts of red meat. Vegetarians and vegans, with the lowest baseline stores, tend to see the largest response.
